About the role
The purpose of this role is to provide and demonstrate accountability at an expert practice level for the provision and coordination of patient flow and care delivery from point of entry through to discharge from any of the MNMH sites. In collaboration with the MNMH leadership team the role will lead and support the service improvement agenda, engage with internal and external health care providers to plan and coordinate a complex set of interdependent resources required to facilitate the provision of high-quality patient care across the continuum of care.

The role will:

  • Provide contemporary operational, professional and strategic leadership for patient flow activities within MNMH.
  • Work strategically and operationally with each of the MNMH sites to lead consumer, family and carer orientated activities to support contemporary patient flow.
  • Be a central contact point of contact and escalation within MNH (including PACH) to support safe and timely patient flow activities across the MNMH sites.
  • To support MNMH in the development of patient flow data analytics to respond to the patient flow needs of the patient population.

Appointment to this position requires proof of qualification and registration with the Nursing and Midwifery Board of Australia (NMBA) as a Registered Nurse. Certified copies of the required information must be provided to the appropriate supervisor/manager prior to the commencement of employment.
